Fairfield Hospital Rehabilitation Ward

Patients admitted to our Rehabilitation unit benefit from a multidisciplinary approach to improve their quality of life and increase their activities of daily living. Our team consists of physicians, nurses, physiotherapists, occupational therapists, speech pathologists and social workers.

Aged Care Services Emergency Team (ASET)

Our team provides comprehensive assessments for frail older people who present to the Emergency Department, ensuring their immediate clinical needs are identified and addressed. We offer information, guidance and referrals to support older people, their carers and families in accessing services that best meet their individual needs.

We also advocate for timely access to appropriate care and support options to enhance safety and wellbeing. Wherever possible, we develop a clear plan of care that can be used by inpatient teams and community providers, helping ensure coordinated follow-up and ongoing support.
